ICES Overview

ICES houses anonymized patient data from administrative sources that is linked and analyzed using unique identifiers. Data include physician and lab billings, cancer care databases, hospital and emergency department databases and more. The CareCanvas team worked with Dr. Rick Glazier and ICES staff to prepare and analyze data on quality of care for all 14 core family medicine teaching sites at the University of Toronto. Site leadership defined which physicians were part of their teaching site and should be included in the denominator. We analyzed data for all rostered patients for those physicians (n=311,038). Quality of care indicators were calculated every 6 months for the period from September 30, 2018 – March 31, 2022.

Patient Experience

The patient experience survey is a collaborative effort among 14 core family medicine teaching sites at the University of Toronto to understand what is working for patients and what needs to improve. The survey is comprised of a common core set of questions and all sites adhere to a common distribution process. Surveys are distributed by each site to registered patients via a digital platform (email or secure messaging) every six months. Responses to the common core questions are collected and analyzed centrally at the DFCM and results are provided back to the sites summarizing site-specific aggregate data and DFCM aggregate data including cross-site comparisons and trends over time. We analyzed data for all patient responses from sites participating in the Winter 2022 survey. Data was collected for the period from May 2023 - July 2023.

About CareCanvas

Family physicians across Ontario have come together to design a tool to help primary care clinicians and teams better understand and care for the communities we serve.

CareCanvas is a collaboration between the University of Toronto’s Department of Family and Community Medicine (DFCM) and the Primary Care Ontario Practice-based Learning and Research Network (POPLAR). POPLAR is an initiative of Ontario’s six University Family Medicine departments and the Alliance for Healthier Communities.

CareCanvas is an interactive web-based dashboard that summarizes clinical information from your practice EMR to make it easier to care for your patients. All data comes from your electronic medical record. The data are extracted from your clinic’s information system and cleaned, processed, and stored in a secure server with POPLAR.

Please keep in mind that the data are not perfect. We use algorithms that have been developed by researchers to identify common chronic conditions and some patients may be incorrectly classified.
